Hypnotherapy survey

HI everyone,
I have compiled a survey in order to establish the working life of a hypnotherapist such as income, areas of business, branding, referral rates, website performance and many other issues. These results will be posted online for the benefit of all. It is my intention to establish how therapists work whether full time, part time, NHS referred or stage hypnotists etc. Before the survey goes live I'd appreciate feedback on further questions you would like to know about. I may have missed some.
Please give me your thoughts and I will add to the survey. Obviously all participants will be anonymous.
I hope that eevryone will partake in the survey when it is finalised next week.
