The principle of informed consent says that we, before doing professional hypnosis:

1.)  Get consent

2.)  define the services we offer

3.)  Accuratly represent the limitations of our techniques or training

4.)  Inform clients of other options.

I use a specific form with all of my clients, and have posted a .pdf of that form in the www.ICBCHForum.com group.

Many licensed mental health professionals are REQUIRED to have this written form.   I think, licensed or unlicensed, required or not, it is good policy to have this on file.

in the recent case of the High School principle charged with practicing therapeutic hypnosis, such a form (signed by a parent) would have solved all of his problems.

If you live in a state with strict regulation of terms or that limits the scope of practice unlicensed hypnotists can engage in, this type of form is ESSENTIAL.

In Washington State, a hypnotherapist must either be a Registered Hypnotherapist (with the Department of Health, Division of Professional Licensing), OR he/she must be a licensed health care practitioner practicing within his/her scope of practice.

The Registered Hypnotherapist is required to provide a disclosure form that includes specified language, including your training, experience, fees, etc This form must be signed by BOTH client and hypnotherapist, with a copy given to the client.

However, I believe such a consent and disclosure form is the professional thing to do; and should be done regardless of whether your local law requires it.

I think it shows responsible behaviour on our part to be honest. It is also a back up if you get a funny client who wants to try and sue people !!

I use a client intake form and give them a welcome letter that explains all that you mentioned. I also ask if they are ready to be hypnotized before I start.
