I have a client coming in who wants to lose about 20 lbs. She is 45 yrs old, has true low thyroid and takes meds for that, has a back injury and often times wears a brace. She says she has changed her diet habits and has managed to maintain where shes at now...but is having trouble losing what was already put on due to the injury/thyroid. I'm not sure where to go on this. Any suggestions?

I have worked with hundreds and hundreds of weight loss clients and many seem to have some kind of thyroid problem and many claim to be eating correctly.
However, many thyroid problems are often caused by so called healthy diets. For example soya is "supposed" to be healthy but if you do any research you will find that soya protein is a waste product from the soya oil industry and because it can't be fed to animals or dumped in landfills in has been rebranded as a health food and is added to many processed foods and is even in chocolate. Soya has been proven to disrupt the thyroid function and many of my clients have been able to stop meds (and lose weight) simply by avoiding soy.
Secondly, low fat/high carb diets can mess with the thyroid as can many weight loss "magic pills".
Using hypnosis and ideomotor signals it is possible to get the thyroid to return to normal functioning (see Mental Therapuetics, by Theron Q Dumont)
I think if you really want to have long term consistant results with weight loss clients you have become an expert on nutrition. This is not an easy thing to do as the vast majority of nutritional advice is rubbish. A good place to start is a book by Hannah Sutter, called Big Fat Lies another is called The Diet Delusion by Gary Taubes.
I would also want to know more about the back problem. Many back problems can have a psychological/stress component. For more info see, Healing Back Pain by Dr John Sarno. If she has a lot of stress going on in her life this will effect her ability to lose weight. High cortisol levels can cause weight gain.
As she only needs to lose 20 pounds once you have ruled out or dealt with the above then simply changing her food preferences and having her develop a full feeling in her stomach sooner than usual should do the trick.
I certainly would not want to use Gastric Band hypnosis as this really should only be used for people who are seriously obese. In the same way that gastric band surgery is only for clinically obese then I believe gastric band hypnosis should be reserved for the same type of clients.
I know gastric band hypnosis is the hot new thing (and yes it works) but hypnotherapists have successfully been helping people lose weight for decades with simple behavioral change techniques and I agree with other posts by Jonathan Royle about this process and the safeguards that need to be taken.

I deal with back pain relief on a daily basis as a Doctor of Chiropractic.
Of course, the client is probably presenting due to pain and resetting pain tolerance or perception is crucial. However, pain does not occur without a loss of function (physical, neurological, psychological). The big thing here is to think about function. How can I get this person moving and able to do their activities of daily living (ADL's)? How can we motivate them to get the care they need (best practices demonstrate spinal manipulation and rehabilitation exercises are most effective)? How can we get them to think about nutritional supplementation (glucosamine sulfate, chondroitin sulfate, MSM, Hyaluronic Acid, drinking enough water) and get advice from a qualified provider? How can we help them over the fear of pain during movement (explaining the difference between pain and injury)? How can we reinforce proper movement/transferring (getting in and out of bed/cars, etc) How can we help inspire or motivate them to do the stretches, exercises, and maintenance they will need? How can we decrease the fear of flare ups, as we recognize these will occur.
I suspect my perspective may be a bit unique. I hope it helps.

The only thing I would add is to check in with the client at all levels to be sure that this is the most pressing issue for the client to solve now. If there is another more urgent emotional issue, the weight loss goal might be suffering.

it assumes we (our bodies) are designed to eat as cave-people ate....
sometimes berries for days
then meat for days
then roots ...for days
etc etc
NEVER combining eg carbs and meat, the body cant handle that ... and isnt built to do this... these assumptions clear, now for all the suggestions...
when you want meat, you will not eat rice, pasta etc carbs with it, you will choose vegetables
when you eat carbs potatoes rice etc you will NOT eat meat with it
now this is a certain way to lose weight but needs dicipline. Hypnosis facilitates this ... the client will not combine in the wrong way, its that simple ...
