I have been asked by an insurance company to submit any and all medical records for a client for processing an insurance claim.
I do not hold a counselor's degree and have never been asked for these records before. If anyone can help me with what exactly to provide, that would be great. They have included the HIPPA release form
(I did explain to the lady who called from the claims company that I do not diagnose, as I do not have a medical license).

Permalink Reply by Fable Goodman on September 20, 2011 at 7:43am I am wondering if the Insurance claim, has anything to do with what you are working on with the client?
ie, is she attempting to get her insurers to pay for her treatment.
If this is the case, then they probably have a right to ask for whatever record you keep, of how many sessions, how much, and what you are treating the client for. but these are not medical records.
You will need to agree with the client, just what disclosures they are happy for you to make.
Do not disclose client information without explicit permission from the client.

don't disclose anything unless you have written consent from the client. I'd clarify with them if they just want intake forms and dates of sessions rather than your notes.
I've submitted my paperwork to third parties before and it's basically useless to them (I keep notes in a way that really only makes sense to me, but other hypnotists could figure out parts of it).

I do have the signed HIPPA release form, but I agree it would be a good idea to speak with her about what is okay to share with them. Intake forms and dates of sessions might be more helpful anyway.

My guess is your client was/is trying to get reimbursement for your services. I recommend confirming that with your client and see how she indicated that you helped her. FYI - My understanding is the signed HIPPA release form was signed by your client and authorizes you to respond to the request for more info.
FYI- I have been asked to do this several times in the past and I responded by writing that I do not keep medical records because I am not a licensed health care provider and I do not provide medical services. I then indicate that I am a medical hypnotist and I teach clients how to use self-hypnosis for stress reduction, pain relief or to change unwanted behaviors, etc..
I included date and time of the appointment - My fee and a very brief comment
Example:
Client X:
Tues 2/12/11 2PM Self-hypnosis training for pain relief - $150. Paid in full
