I have a female client, age 43, who has just started learning hypnosis via a CD I made for her and a live session (1 thus far).  With both she demonstrates trance (eye test, leg or arms heavy, arm catelepsy) and then when she starts to really "go deep" and in her words "when everything else seems to go away" she has to pee -- URGENTLY!  When she gets up there is very little in her bladder.  *Has anyone experience this before?* 

She is a very motivated subject and we have discussed prime directives of the UM and possible causes.  She is even willing to wear "protection" so that she can allow herself to "go" during the session rather than get up!  She has no waking problems with urination or bladder control of any kind in any other scenario.

I had hosted a medically referred session years ago with a woman, whom had brought her husband along, who amongst other things, claimed to have severe OCD. She kept on disappearing to the restroom to adjust her bra. Glad her husband was there. Finally, he suggested she just take it off, she refused. I recall the session well as she had stalled so long, the sun went down and I had to relay the second half of the suggestions in the dark, as I was mic'ed and couldn't reach the lights. I've made sure that would never occur again since.
Generally speaking, what is her payoff? What is she achieving or not achieving by behaving this way?
There may be a few reasons for this reaction. She might be subconsciously afraid of the process or the lack of control some might perceive and this is a take control back defense mechanism.
It might be she need to feel a bit "alone" or maybe as I think of it, it might have some link to her past emotionally.
My best suggested approach would be to Speed-Trance her and quickly give her a suggestion about forgiving, releasing and letting go of (no puns intended here) of this habit/reaction, in ways of profound and truest impact, regardless of where this came from, in ways both known and unknown to her, and while she's in trance, ask her to nod her head yes, as she now knows this ability relax comfortable, free of restroom responses, to both now and forever, be real and true (subconscious agreement).
I cannot speak directly to your client's issue, however I have had clients have to use the bathroom during the session. I merely explain to them that I will count from one to three, and on the count of three they will open their eyes ... understanding that they are still in hypnosis ... and that when they return to the chair, they will immediately return to the same level of hypnosis as before.

They then get up, use the restroom, come back to the recliner (normally smiling and laughing) ... I then explain I am going to count from three down to one, they close their eyes on the count of one, and within a few short seconds return to a very deep level of hypnosis.

You did not give us an indication of her medical condition ... but I would assume she may already have a bladder control issue, is self-conscious of it, and when she relaxes it probably triggers anxiety (conscious mind) of wetting her pants ... kind of like just before you get on an airplane or going on a roadtrip somewhere - you don't really have to go, but that nervous sensation in your stomach triggers those butterflys and there is a sensation of having to use the restroom.

If there is an anxiety issue, then perhaps instead of having her wear protection, you could spend some time addressing the issue, then future pace to a time where she sees / feels herself doing just fine.

On a more humorous note ... I hope your recliner is scotchguarded :)
