Some of the forms you'll need depend upon where you intend to practice. For example, the legal forms needed in California, such as a CAM disclosure, and keeping it on file for three years, may not be needed where you practice.
I imagine you'll find various induction forms, disclaimers, etc., here. You might also go to your regular physician's office and ask for a copy of his or her forms, and modify them to meet your needs.

I am new to hypnosis but love it all ready. Can some one direct me on what intake/informed consent forms or other forms I may need and where I could find them?
