Hi, Lela.

Some of the forms you'll need depend upon where you intend to practice. For example, the legal forms needed in California, such as a CAM disclosure, and keeping it on file for three years, may not be needed where you practice.

I imagine you'll find various induction forms, disclaimers, etc., here. You might also go to your regular physician's office and ask for a copy of his or her forms, and modify them to meet your needs.

Welcome, you new members! I'm glad you like the group. I know I wish I had access to some of this when I first started out and even after nearly 10 yrs, I am gleaning treasures here...
