I need to preface this post with the statement that I do not diagnose. That said, I have client who has presented for motivation, spiritual issues, guilt issues, anger, "my wife says I'm depressed" and then it went on and on from there. Repeatedly during our initial interview I had to pull him back to the subject and remind him what the question was.
I believe this client to have a relatively mild undiagnosed bipolar disorder simply from what I witnessed (flight of thoughts, difficulty staying focused) and what he described as depression (lack of motivation, feeling sad and down, dwelling on past mistakes). Important: he occasionally thinks "why do I bother to live?" but denies any compulsion to act out SI ever.
The initial plan is to deal with his guilt and childhood issues. I would also like to address what I think are bipolar symptoms. My first thought, being that he is a drummer, was a guided imagery using drumming as a metaphor for controlling the tempo and rhythm of his thoughts. However, he is unaware of his (presumed) 'manic' state.
Any thoughts or suggestions?

bit late, but I just saw this post.
You know, diagnosing is for the other guys, the psycho's.
They deal with the conscious mind, we use the SC.
And the nice way of our way of working; we do not have to search for answers!
We have a great answering machine: the SC!
Do not assume you know what the cause of a problem is, just ask 1 question, always:
"SC, take me now to the real and only cause of this particular problem".
There, that's it.
Bipolar disorders, ADHD, etc, they do not exist.
They are merely ways of dealing with problems from the SC.
Find the cause (situation/persons), fix that, symptoms will disappear.
And if they don't: you didn't find the real cause, start over.
