I'm a dentist, and I have a young female (late teens to early 20's) coming in this week for a dental examination and dental hygiene appointment. Her mother informed us when she made the appointment that her daughter has POTS, which has forced her to take a leave of absence from college.  her mom said that the physicians have tried many different treatment regimes, all of which have not been effective.  She has been through physical therapy and accupuncture, none of which have been effective.  As I understand it, the primary symptoms seem to be hypotensive crises with tachycardia (i.e. low blood sugar with a racing/irregular heartbeat).  It seems to me to be the opposite of a vascular migraine, and it therefore seems that a hypnosis/biofeedbck approach should be helpful. 

   If using hypnosis and biofeedback can be used to cause peripheral vasodilatiation, and therefore lower intracranial pressure; it shoudl be feasible to use the reverse technique - to try to get peripheral vasoconstriction to increase cerebral pressure.  I apologize if I'm sounding too clinical, but I've spend a lot of time trying to think this out.  What do you think? (I'm also going to post this in another group to see if i can get other views).  Thanks for your viewpoints.

I am not sure if you are a hypnotherapists or not, but assuming you are, there are many ways you can help your client to improve her situation.

 

You can start with finding out what is her eating habits and via hypnosis give her suggestions to eat healthier food to help her relieve hypotension, since clients with POTS must eat frequent small meals instead of three large meals a day, and by doing that, it will help her to reduce the amount of blood needed for digestion. That means, more blood for her brain and heart.

 

Also make sure to give her suggestions to refined carbohydrates, such as white flour and sugar, can exacerbate hypotension. You may have to talk to her doctor to find out what is important for her to identify and avoid her food triggers, some POTS clients need to stay away from dairy products.  Also recommended suggestions to tap in your clients subconscious mind is to reduce stress and anxiety, to counter maneuver her movement to help decrease symptoms by lessening the amount of blood that pools in her legs.

 

Here are some useful suggestions:

 

1. Excercise; Please investigate what type of exercise can be helpful for her, and from what I have learned, the tightening and building the leg muscles will help her to squeeze pooling blood back to her upper part of her body.

 

2.Stand with her legs crossed.

 

3.To sit in a low chair.

 

4. Sit with the knee to chest position.

 

5. Lean forward with her hands on her knees when sitting and tightening her buttocks, thigh and leg muscles especially when she is standing for any length of time. ( This action of tensing the leg muscles while standing enhances brain blood flow and reduces sympathetic activity.)

 

6. Before and during hypnosis give her a suggestion to elevate her head by putting extra pillows under her head or to elevate the head of her bed between 4 to 12 Inches, or perhaps you can ask her physician to help you understand what is better for her.

 

7. Swimming in water, but not recommended to go into the water by herself.

 

8. 20 minutes a day aerobic exercise at list three tomes a week can be great if she can do it.

 

9. Drinking enough water can help POTS client to feel better, since it has been shown to moderately reduce orthostatic tachycardia in clients with idiopathic orthostatic intolerance, especially drinking large amounts of water does help to raise blood pressure as well as increasing blood volume, which is especially useful in the hypovolemic and those with pooling blood. (Physicians suggest that their patients drink eight ounce glasses of water daily (Low, 2000). But they must not drink excessive amounts of water because doing so can cause essential electrolytes to become diluted in the bloodstream, which may affect heart rhythm. 

 

11. Help her increase her salt intake to no salt if possible, I am sure you know that salt helps to expand blood volume. Number of clients find salt tablets and/or electrolyte solutions to be helpful. Some physicians suggest their patients who benefit from salt take in 10-15 grams daily (Low, 2000). Other physicians suggest between 3-5 grams of salt per day. Salt is 39% sodium, therefore 15 grams of salt equals 5,850 mg of sodium. Again consult her pysician. 

 

12. Hypnosis to boost and Improve Emune System.

 

13.  Hypnosis for Insomnia, Deep Sleep, since POTS clients need pplenty of rest due to their serious sleep disturbances, and they need more sleep than the average person.

 

Any ways this is how I would offer help to your client, and if you have any questions and If you are not a hypnotherapist, I will be happy to help by communicating with your client and with her pysician.

 

PS: I am not a doctor, so please make sure to talk to her pysician and this information is only intended as an additional help and not for advice from a health care provider. All what I suggested must be checked and confirmed by her psycian first with  possible treatment options from her doctor. And her hypnotherapy sessions must be with her doctor's refferal and probabley even his or her supervision.

Doreen, thank you for your response.  Your answers are accurate, and I am familiar with almost all of them;  My question was more about the biofeedback/guided imagery approach.  I am IACT certified, and Mike Ellner has been one of my main gurus - I know he's recommended a guided imagery approach for migraines; I'm interested in seeing if the "opposite" of migraine/vasodilation could be effective for this POTS problem.  What's your opinion?

Hi Steve, To be honest, I don't know!

All I can think of, is maybe, asking your clients subconscious mind if this approach will be a possible approach to hep her, by asking her subconscious to raise one of her fingers so you know, chose which finger will be the yes finger, the No finger and the, am not sure or I don't know finger and follow through, this way at list you are communicating with the subconscious mind for going forward, and I believe if no one done it yet, this will be your red light or green light to give it a chance and see what happens?

Usually when it comes to hypnotherapy, I say if it doesn't help it can;t kill you, so attempting to help maybe be a great way to find out, and if you want, you can let your client know that you are going to ask her subconscious mind first, since some people reject certain healing modality and some except it, with the hope that her mind will be open to to the new approach

Hello Steve,
just a short note to say hi and see how the progress may have been with your patient. I use an EEG for biofeedback. It allows me to
not only prove that the brain waves are changing from hypnosis. It additionally allows me a visual indicator when suggestions may be most appropriate to mention, for example, when brainwaves are in the theta state.
