at present im writing a sports hypnosis intake form so that i can have some detailed information about the client and there issues before they come in for therapy.

I have done an intake form for various other issues like stress weight loss smoking etc

but i was wondering what would you ask a client with sport related issues?

Sports questionnaire

What is your goal for therapy?

Why have you decided to come to a hypnotherapist?

What aspects of your sport are you good at? Bad at?

How hard do you think it would be to achieve your goal? (On a scale 0-10)

What support do you have to achieve this goal?

What areas are you in control of and not in control of relating to your goal?

What aspects of your sport do you want to improve?

Have you ever used hypnosis or anything similar to help you before?

Do you know any one who has the skills you would like to have?

What stops you performing the way you would like to?

What’s it like when you’re performing at your worst?

What’s it like when you perform at your best?

How would you know you were performing the way you like to? (What would you see, hear, feel?)

Do you have a training program? (Give details)

How do you deal with stress?

Are there any areas in your life you feel unconfident?

Are there any areas in your life you feel confident?

What have you learnt about your self from having this problem?

What level do you want to reach?

Why is achieving your goal important to you?

What one thing would I change for the better? If I could only change one thing?

If you woke up in the morning and all of your problems were gone what would be different?
What changes would you notice?
What would you feel like?
How would you know that the problem is gone?

Is there any thing else you would like to tell me about your issue?

You have to be able to run fast to catch your prospective client as he runs away from you in shock after reading that lot! keep it simple and keep your detailed questions for the consultation!!!!!!
I would keep it very simple to start with, perhaps just asking them to outline what the main problem is, and perhaps completing a 10 or 15 point questionnaire rating their various mental skill levels, e.g. concentration, ability to relax, pre-comp nerves etc. The rest can be left for the actual consultation where it's probably more suited to greater detail. Although it would be interesting to see what your clients made of this questionnaire - it may mean you can get to work much quicker when you first see them, which they may appreciate?
At the risk of being trampled I'll reveal that am a Sports Psychologist working mainly with hypnosis. I agree with Gary in that you should keep it simple. Assessing the anxiety levels of a sportsman/woman is a good idea and for that I use a SCAT test. Controlling anxiety is the key to many issues in sports as well as realistic goal setting and aspects such as "appropriate focusing." In order to work effectively with sports clients you need to know something about the psychology of sports performance. In general, once you have the basics you can be very helpful to your sports clients.
