There are several good references for documentation but I do not have any of them here at home with me. I ran across another one just last week but can't remember for the life of me where it was published. Try Cochrane or PubMed.com. Those are the two search engines I use most often so chances are that I read it in some article in one of them. I can't remember which topic I was researching at the time--probably fibromyalgia. I do know that it is well documented in multiple papers even if I can't remember which ones. Sorry.
