Quick Quiz for you therapists

This is my second attempt, originally I thought I could embed the quiz in the forum but that didn't work. Those of you working in the therapy field, if you don't mind follow the link below and answer 9 quick questions for me. I'm collecting some statistics to use in a business plan model. The more data the better. Thank you in advance.

I filled it in anyways because I just finished a business plan and I can understand where you are coming from...that's not the easiest thing in the world to complete. Best of luck to you.
The google docs thing is a really cool idea......I never thought of that. Thanks!

Permalink Reply by Jonas on August 20, 2008 at 1:36pm: yeah, google docs is awesome. it'll dump all the answers into a spreadsheet for me automatically, and i can generate graphs and charts and stuff easily. wasn't thinking about you wonderful canadian folk, i'll go add an option for in the future (but you need not worry about taking the quiz again)
